Do I need a referral for physical therapy?
In California, you do not need a referral from a medical doctor to see a physical therapist. With “direct access,” you can schedule an appointment with us at any time.

Does Revive Roots take insurance ?
We are an out-of-network provider, which allows us to deliver the highest quality care without the restrictions imposed by insurance networks. If you have a PPO plan, we can supply you with a superbill—a detailed receipt—for you to submit to your insurer for possible reimbursement; the reimbursement rate typically varies from 30% to 90%, depending on your plan specifics. To determine your exact coverage, please contact your insurance provider. You may also use HSA and FSA accounts to pay for services.
